Please email, fax, or mail this sponsorship commitment form along with your payment to YOS by May 17, 2024. Complete the form and return it to Beth Spallone Director of Philanthropy at beths@yos.org. Or send by mail to Youth Outreach Services, Attn: Philanthropy Department, 2411 W. Congress Pkwy., Chicago, IL 60612.
We offer various sponsorship levels ranging from $500 to $10,000 to fit every budget and several options to make your donation.
CHECK: Make check(s) payable to “Youth Outreach Services” and mail or drop off to a Youth Outreach Services admin office (same as above).
EMAIL: Email a copy of your commitment form to Beth Spallone beths@yos.org or Fax: 773-777-7611.
ONLINE: Pay with your credit card. To avoid processing fees, send or email a copy of your commitment form to Beth Spallone @ beths@yos.org and we will process your transaction manually.
