Please email, fax, or mail this sponsorship commitment form along with your payment to YOS by October 30, 2026. Complete the form and return it to Beth Spallone, Director of Philanthropy, at beths@yos.org. Or send by mail to Youth Outreach Services, Attn: Philanthropy Department, 2411 W. Congress Pkwy., Chicago, IL 60612.

We offer several sponsorship levels ranging from $500 to over $10,000, accommodating every budget, along with multiple donation options.

CHECK:   Make check(s) payable to “Youth Outreach Services” and mail or drop off to a Youth Outreach Services admin office (same as above).
